For the final stage of the Healthy Communities process, it is essential that a systematic evaluation process is established to monitor the quality of the intervention strategy and the impact it has on the community. The lead organization and the local representatives jointly plan the monitoring process with input from community stakeholders, while participants are responsible for monitoring the implementation strategies and gathering outcome data.

Once a specific intervention has been identified for the targeted community, a set of indicators are defined to measure relevant expected outcomes. For example, should the community choose to implement a domestic violence program, one outcome indicator might be the percentage increase in number of victims of domestic violence receiving counseling and/or legal services. Indicators are selected that are clearly measurable within the local context and that are directly attributable to Healthy Communities project activities. Information gathered during the community assessment (phase 3) provides baseline data for the development of and measurement of progress and success against performance indicators.

The information gained through the monitoring and evaluation process is useful only if it results in positive adjustments and changes to the programs being implemented. This may also lead to the development of new community interventions to address newly identified needs.

Objectives

  1. Community members and stakeholders understand the importance of implementing quality monitoring and evaluation processes
  2. Develop performance indicators for community interventions
  3. Collect relevant information and data and analyze results
  4. Share results with the community team and adjust the interventions as necessary
Expected Outcomes:
  1. Monitoring and evaluation are integrated into the local Healthy Communities process
  2. An evaluation report is produced documenting successes, lessons learned, and implementation challenges of the selected intervention
  3. Interventions are adjusted and new ones developed based on monitoring and evaluation results
