- Initial Assessment
When AIHA partners in the United States first met with their counterparts in other nations, the groups convened and filled out this assessment. This comprehensive document covers a range of topics that may also be relevant to your work.
- Focus Group Questionnaire
Youth risk behaviors affect most regions and nations and this questionnaire deals with this topic. But even if youth risk is not something you wish to research at this time, you may still wish to use and adapt this document.
- SWOT Analysis

Measurement and Evaluation Resources
- Primary Healthcare Center Model Ideas
This resource is a chart, outlining a variety of innovative and valuable ways to establish the measurement and evaluation phase. The heading asks the question, “How can these criteria be measured at your institution?” and topics covered include: counseling; involvement of nurses in education activities; availability of patient education materials developed through partnership; group health education/promotion classes for patients; involvement of nurses in direct patient care; screening services; continuous quality improvement; expanded skills of Primary Healthcare Center physicians and nurses; occupational health and infection control practices in place; and community outreach.
- Health Facility Monitoring and Evaluation Assessment
This four-page worksheet was designed to help gather data in a number of focus areas. These areas include: service delivery; data collection; supervision of data collection; data management; data analysis; reporting of aggregated/analyzed data and evaluation.
- AIHA Knowledge, Attitudes, Beliefs, and Practices (KABP) Participation Survey
This briefing and decision document summarizes the results of the formative phase of survey development and outlines the plan for the AIHA survey.
- Patient Satisfaction Survey
This survey is straightforward and easy to use. Patients are asked to rank their satisfaction level on a number of issues, allowing them a broad range of responses from very dissatisfied (1) to very satisfied (7). Patients are reassured that their responses will remain anonymous.
- Client Exit Interview
The client exit interview document is similar to the patient satisfaction survey, but differs in that the participant does not remain anonymous, but speaks directly to an interviewer who records the patient’s responses.
- Breast Health Workshop Pre- and Post-Test
This document, designed specifically to measure learning related to breast health, could be adapted to fit any workshop topic. The findings allow you to compare the level of knowledge a participant has before and after the workshop, and can help you determine how effective your workshop presentation has been.
